How it works

Ideal body weight (IBW) is a reference weight derived from your height and sex. It is used every day in medicine — to dose drugs such as aminoglycosides, to set ventilator tidal volume in millilitres per kilogram of IBW, and to normalise creatinine clearance — because dosing on a very heavy patient's actual weight can overshoot. All four formulas on this page share the same shape:

IBW = base + perInch × n, where n = (height in inches) − 60

The height is first converted from centimetres to inches (dividing by 2.54), then 60 inches — five feet — is subtracted to get n, the number of inches over five feet. Each formula plugs its own base weight and per-inch increment into that equation:

  • Devine (1974): male 50 + 2.3n · female 45.5 + 2.3n
  • Robinson (1983): male 52 + 1.9n · female 49 + 1.7n
  • Miller (1983): male 56.2 + 1.41n · female 53.1 + 1.36n
  • Hamwi (1964): male 48 + 2.7n · female 45.5 + 2.2n

Devine is flagged as the dosing standard because most dosing research and guidelines are built on it. The tool also computes the WHO healthy weight range by evaluating weight = BMI × height² at BMI 18.5 and 24.9, giving a low–high band in kilograms for your height.

When you enter an actual weight, two more numbers appear. Adjusted body weight (ABW) = IBW + 0.4 × (actual − IBW), using the Devine IBW; the 0.4 factor reflects the fraction of excess weight that is metabolically active. Percent of IBW is actual ÷ IBW × 100. Clinical convention then picks the dosing weight: actual weight when you are at or below IBW, IBW between there and 120% of IBW, and ABW at or above 120%. Every Devine value is cross-checked against the equivalent metric form of the equation, so the centimetre-to-inch step is verified internally.

Worked examples

Male, 175 cm, no actual weight

  1. Inches: 175 ÷ 2.54 = 68.90 in → n = 68.90 − 60 = 8.90
  2. Devine: 50 + 2.3 × 8.90 = 70.5 kg (dosing standard)
  3. Robinson: 52 + 1.9 × 8.90 = 68.9 kg
  4. Miller: 56.2 + 1.41 × 8.90 = 68.8 kg
  5. Hamwi: 48 + 2.7 × 8.90 = 72.0 kg
  6. Healthy range: 18.5 × 1.75² = 56.7 kg to 24.9 × 1.75² = 76.3 kg
  7. Devine IBW (70.5 kg) sits inside the healthy band.

Female, 160 cm, actual weight 75 kg

  1. Inches: 160 ÷ 2.54 = 62.99 in → n = 2.99
  2. Devine IBW: 45.5 + 2.3 × 2.99 = 52.4 kg
  3. Adjusted body weight: 52.4 + 0.4 × (75 − 52.4) = 61.4 kg
  4. Percent of IBW: 75 ÷ 52.4 × 100 = 143%
  5. BMI: 75 ÷ 1.60² = 29.3 → above the healthy range
  6. Healthy range: 47.4 kg to 63.7 kg — 75 kg is above it
  7. 143% ≥ 120% of IBW → dose on adjusted body weight (61.4 kg)

Male, exactly 152.4 cm (the 5-foot pivot)

  1. Inches: 152.4 ÷ 2.54 = 60.00 in → n = 0
  2. With n = 0 every formula returns its base weight exactly:

  4. Healthy range: 18.5 × 1.524² = 43.0 kg to 24.9 × 1.524² = 57.8 kg
