eAG is a population average from the ADAG study; your true average can differ. The relationship is unreliable with anaemia, recent blood transfusion, pregnancy, chronic kidney or liver disease, haemoglobin variants (e.g. HbE, common in South Asia) or any condition that shortens red-cell lifespan. Always confirm with a laboratory HbA1c and your doctor.

How it works

HbA1c measures the share of your haemoglobin that has glucose stuck to it, which reflects your average blood sugar over the roughly three months a red blood cell lives. In 2008 the international ADAG (A1C-Derived Average Glucose) study measured continuous and fingerstick glucose in 507 people and fitted a straight line between HbA1c and their true average glucose. That line is the formula this calculator uses.

  1. Normalise the unit. If you enter HbA1c in IFCC mmol/mol, it is first converted to the DCCT % scale with the NGSP–IFCC master equation: A1C% = mmol/mol ÷ 10.929 + 2.15.
  2. Apply the ADAG regression. eAG (mg/dL) = 28.7 × A1C − 46.7. This had an R² of 0.84 in the original study.
  3. Convert to mmol/L.Divide the mg/dL figure by 18.0182, glucose's exact molar factor. The paper also published a parallel line, eAG (mmol/L) = 1.59 × A1C − 2.59, and the two agree to within about 0.1 mmol/L — a built-in cross-check.
  4. Classify. The ADA Standards of Care set HbA1c below 5.7% as normal, 5.7–6.4% as prediabetes and 6.5% or above as diabetes, with a general adult target below 7.0%.

The reverse direction simply inverts the regression: A1C% = (eAG mg/dL + 46.7) ÷ 28.7. Every figure is rounded the way labs report it — mg/dL to whole numbers, mmol/L to one decimal, HbA1c to one decimal and IFCC to whole mmol/mol. The conversions are pure and identical for everyone; eAG is a population average, so your personal average can differ, especially with the red-cell conditions listed in the disclaimer.

Worked examples

HbA1c 7.0% → average glucose

  1. eAG (mg/dL) = 28.7 × 7.0 − 46.7 = 200.9 − 46.7 = 154.2 → 154 mg/dL
  2. mmol/L = 154.2 ÷ 18.0182 = 8.56 → 8.6 mmol/L
  3. IFCC = (7.0 − 2.15) × 10.929 = 53.0 → 53 mmol/mol
  4. Band: diabetes range, exactly at the 7% adult target. Matches the ADA table (154 mg/dL).

HbA1c 6.0% → average glucose

  1. eAG (mg/dL) = 28.7 × 6.0 − 46.7 = 172.2 − 46.7 = 125.5 → 126 mg/dL
  2. mmol/L = 125.5 ÷ 18.0182 = 6.97 → 7.0 mmol/L
  3. IFCC = (6.0 − 2.15) × 10.929 = 42.1 → 42 mmol/mol
  4. Band: prediabetes range. Matches the ADA table (126 mg/dL).

Reverse: average glucose 200 mg/dL → HbA1c

  1. A1C% = (200 + 46.7) ÷ 28.7 = 246.7 ÷ 28.7 = 8.596 → 8.6%
  2. IFCC = (8.596 − 2.15) × 10.929 = 70.4 → 70 mmol/mol
  3. mmol/L = 200 ÷ 18.0182 = 11.1 mmol/L
  4. Band: diabetes range, above the 7% target.
