Take with you

  • Go in person, NTMI examines the applicant, so nobody can attend on your behalf.
  • Take your national identity card, or a valid passport that carries your NIC number.
  • A police report for a lost identity card, or a Grama Niladhari receipt for applying for a new one, is not accepted in place of the NIC.
  • No photographs are needed: the certificate photograph is taken during the examination.
  • Take your spectacles if you use them to see long distances.
  • If you are on long-term treatment for any condition (diabetes, hypertension and the like), take the medicine at the prescribed times.

NTMI does its own investigations and will not accept laboratory reports from outside. If you lose the certificate it keeps no copy: you pay again and are examined again.

Booking a slot

Book through NTMI's own appointment portal, call 225 from any mobile or 1225 on an SLT line, or use e-Channelling. Walking in without a slot is still possible, but the portal says to arrive 15 minutes before a booked slot or risk being re-queued.

How it works

A Sri Lankan driving licence is issued and renewed on the strength of a medical fitness certificate, and the certificate comes from one institution: the National Transport Medical Institute. The Department of Motor Traffic collects its own licence levy; NTMI collects the medical charge. They are two different payments at two different counters, and this page prices the second one.

This is a lookup, not a model. There is exactly one piece of arithmetic in it:

total = (foreign-applicant rate ?? service fee) + (X-ray asked for ? 700 : 0)

Everything else is read from NTMI's own tables. The 22priced rows on its Services page are transcribed into the data module behind this tool, and each one keeps NTMI's own row text, “Gov. Institution”, “Route Permit (Omini Bus)”, spelling included, so you can find the figure at the source. The fee breakdown names that row beside every line.

The four rules that decide whether you qualify

NTMI publishes its prerequisites per purpose rather than as one table, so the tool evaluates only the ones attached to the service you picked:

  • Age. 17 completed years for a new light-vehicle licence, 20 years for a heavy-vehicle learner permit, 21 completed years for a heavy-vehicle licence. Each threshold is tested as completed years ≥ threshold, counting from your date of birth, so a birthday you have not reached does not count. If you are short, the tool names the date you clear it.
  • Holding period. For the heavy-vehicle services you must have held a B or B1 licence for at least 2 years before the date of application. Tested as years held ≥ 2, with the shortfall reported in months.
  • Height. The motor bus and motor lorry categories need a minimum height of 5 feet. Feet and inches are converted to inches and compared with 60; centimetres are divided by 2.54 first, so 152.4 cm sits exactly on the threshold and passes. Falling short blocks those two categories only, which is what the result says.
  • Documents. The common requirements apply to everyone, attend in person, carry the NIC or a passport bearing the NIC number, and know that a lost-NIC police report or a Grama Niladhari receipt will not do. Each service then adds its own: the old licence on a renewal, the existing licence on a heavy-vehicle application, the heavy-vehicle licence on a route permit, and the court or traffic-police letter with the four fields NTMI requires on it.

Where a figure does not exist

Two charges have no published number. NTMI prints a dash against “Over 40 Medical Certification”, and it notes that some Government institutions want an ECG on top of the X-ray and that the applicant pays an additional amount without saying how much. Both are named as gaps in the breakdown rather than filled with a guess, because a plausible-looking invented figure is worse than an honest blank.

How the figures are checked

Each service record carries its fee, and a separate ledger carries NTMI's rows keyed by their published text. The tool prices your visit from the first and then re-prices it from the second; the two paths meet only at NTMI's page, so a mistyped figure shows up as a disagreement rather than as an answer. The same check runs over all 22services and over the five worked examples below, and it also asserts that NTMI's two separately published X-ray rows still agree at Rs 700.00.
